Confirm program starting date and duration with APAGE Secretariat
Submit the required documents 3 months (short-term) or 6 months (long-term) before the fellowship begins
Complete membership registration and fee deposit
Apply for a visa after receiving fellowship approvals from the Ministry of Health and Welfare in Taiwan
Required documents
Curriculum Vitae (CV)
A copy of Diploma (Bachelor, Master or Ph.D. in English and notarized if it is necessary)
A copy of medical practice license (in English)
A copy of passport (at least the remaining validation is 6 months)
Two recommendation letters
The latest copy of working experience or certificate (to prove that your working experience in gynecology is more than 2 years, and to be issued within 180 days before the training date)
2×2” color photo (formal)
Health Report in English and must contain the following,
- Re-check Hepatitis B & C (validation within 180 days):
Hepatitis B Antibody (anti-HBs) (validation within 90 days)
- Complete chest X-ray report; X-ray photo is not required and is done within 90 days before the training starting date
- Measles Antibody and vaccination certificate (Measles IgG) (validation within 90 days)
- Rubella Antibody and vaccination certificate (Rubella IgG) (validation within 90 days)
- RPR/VDRL
NOTE
i)
Applicants who do not have Measles Antibody and Rubella Antibody shall get the vaccination for Measles and Rubella. However, applicants who have contraindications shall provide a not-suitable-for-vaccination certificate.
ii) Please examine again and re-submit the health report within 90 days before the programme begins.
iii) The applicants will be responsible for the cost of all additional health examination or injection requested by Chang Gung Memorial Hospital.
A copy of certificate of international “Travel Medical Insurance” in English (issued by any of the insurance companies, the insurance item should be “Travel Medical” insurance and the insurance period should be the period of the training program) The cost of medical care and treatment during the period of training should be covered by the applicant.

The applicants hereby agree to have their names, photos, and institutions announced on the APAGE website by completing the application.
Remarks
Applicants are advised to confirm the funding before applying for a program.
All applicants must have 10-year APAGE membership, and one-year fellows are required to attend at least 2 international workshops during their training. Thus, including the registration fees of the workshops, tuition, and membership, the total comes to USD 5,000. The membership fee for short-term fellows, whose training period is less than three months, is a total of USD 1,000.
If applicants would like to cancel the fellowship after the approvals have been confirmed, please submit an official letter of explanation to the training facility; the applicants will not eligible for re-registration.
